GeriMedRisk is thrilled to announce a new pilot program offering a Best Possible Medication History (BPMH) service to clinicians. This service provides accurate and comprehensive medication histories for your patients before their in-person appointments, enhancing patient care and optimizing consultation time. We invite clinics to collaborate with us in this pilot project.

How the PROS BPMH Program Works

 

Inform Consent Icon

Clinic Informs and Obtains Consent from Patient: Clinic staff notify the patient or Substitute Decision Maker (SDM) about the upcoming BPMH appointment and obtain their consent for GeriMedRisk to contact them.

Schedule Appointment Icon

Clinic Schedules BPMH Appointment with GeriMedRisk: Specialist clinic staff identifies patients who need a BPMH and book a BPMH appointment with GeriMedRisk using the PROS Booking App at least 3 days before the patient's clinic visit.

Conduct BPMH Icon

GeriMedRisk Conducts BPMH: The GeriMedRisk pharmacy team contacts the patient/caregiver to perform the BPMH, including a one-year lookback of their medication history.

Fax Note Icon

BPMH Note is Faxed to Clinic: The BPMH note is sent to the clinic within 48 hours of the BPMH appointment, providing specialists with comprehensive medication information before the patient's visit.
